Job Description
POSITION SUMMARY
The primary function of this position is to provide transportation services to all eligible clients (Adults) of The Guidance Center requiring transportation for a TGC appointment.POSITION DUTIES
- Pick up Clients at the designated pick-up point and transport to TGC facilities for their scheduled appointments. Transport Clients back to their destination after the scheduled appointment.
- Document the mileage driven and bill for transportation daily.
- Complete and maintain the Transport log.
- The employee must work well under pressure meeting multiple and sometimes conflicting deadlines.
- The employee shall at all times, demonstrate cooperative behavior with colleagues, supervisors and clients.
- The employee must be able to work a flexible schedule as needed.
- Adhere to all federal/state regulations and safety guidelines as they pertain to operating a vehicle and providing NEMT.
- Explain the required Attestation form to all members before transport and obtain the necessary signatures.
- Adhere to the daily schedule as provided by the Member Services Manager or designee.
- Transportation of anyone that is not a client of TGC with an appointment or an employee of TGC is forbidden.
- The employee shall demonstrate safe driving at all times and obey all traffic laws.
- Other duties as assigned.
GENERAL EXPECTATIONS
- Provide high-quality customer service to both internal and external customers.

- Participate in Quality Improvement and Performance Improvement processes.
- Complete all required training on time.
- Maintain a high level of professionalism while performing job functions.
- Demonstrate flexibility and willingness to learn.
- This position is 100% onsite/TGC campus, in the state of Arizona.
EDUCATION/EXPERIENCE
Required:
- High school diploma or high school equivalency diploma.
- Must be at least 21 years of age.
- Current driver's license and ability to pass MVR screening.
- Must be able to obtain and maintain an Arizona Level One Fingerprint Clearance Card.
- Must have the ability to pass the TGC driver background screening.
- Must adhere to the confidentiality standards of the organization.
- Must be able to drive a 15-passenger van.
- Must be able to drive in adverse weather driving conditions.
