Job Description

Littleton Care & Rehabilitation Center is seeking a compassionate, organized, and team-oriented professional to join our leadership team as the Clinical Transitions & Health Information Coordinator. This unique hybrid role combines discharge planning, care coordination, and medical records oversight within our 35-bed skilled nursing and rehabilitation community. We are looking for someone who thrives in a fast-paced healthcare environment, communicates effectively with residents and families, and takes pride in creating smooth transitions of care while maintaining strong regulatory compliance. Responsibilities Coordinate resident discharge planning and transitions of care Communicate with residents, families, hospitals, physicians, and community partners Coordinate with insurance providers and managed care organizations regarding authorizations and coverage Collaborate with nursing, therapy, MDS, and providers to ensure appropriate utilization of skilled services Provide weekly clinical updates to insurance companies to support continued skilled stay authorization Participate in interdisciplinary team meetings and care conferences Provide exceptional customer service and resident advocacy Manage resident medical records and ensure compliance with state and federal regulations Maintain organization of charts, physician orders, audits, and records requests Assist with admissions and insurance documentation processes Support survey readiness and regulatory compliance efforts Collaborate closely with nursing, therapy, admissions, and leadership teams Qualifications Experience in skilled nursing, healthcare, social services, case management, or medical records preferred Strong communication and organizational skills Ability to multitask and prioritize effectively in a fast-paced environment Knowledge of Point Click Care systems and healthcare documentation preferred Experience with discharge planning or insurance coordination is a plus Positive, compassionate, and team-oriented mindset Why Join Our Team? Pay:
$25.00 - $34.00 per hour
