Job Description
Position Summary The Transportation Driver / Appointment Coordinator is responsible for coordinating resident appointments, arranging transportation services, and safely transporting residents to and from medical appointments, procedures, and approved outings. This position serves as a key liaison between residents, families, healthcare providers, and transportation vendors to ensure residents receive timely access to necessary services. The ideal candidate is organized, compassionate, dependable, and committed to supporting both residents and fellow team members. Essential Duties and ResponsibilitiesTransportation Safely transport residents to and from appointments, procedures, hospital visits, dialysis treatments, and approved outings. Assist residents with entering and exiting vehicles while ensuring safety, dignity, and comfort. Properly secure wheelchairs and mobility devices during transport. Conduct routine vehicle inspections and report maintenance or safety concerns. Maintain transportation logs and required documentation. Appointment Coordination Schedule, confirm, and track resident appointments with physicians, specialists, hospitals, clinics, and other healthcare providers. Coordinate schedules to ensure transportation and appointments are managed efficiently. Communicate appointment details with residents, family members, nursing staff, and other departments as needed. Maintain accurate appointment calendars and resident transportation records. Follow up on appointment changes, cancellations, referrals, and new orders. Outsourced Transportation Management Coordinate transportation through outside vendors when facility transportation is unavailable or when specialized transportation is required. Ensure outsourced transportation services are scheduled timely and are appropriate for resident needs. Communicate appointment details and transportation requirements with vendors. Monitor transportation arrangements and resolve scheduling issues as they arise. Teamwork and Resident Support Foster positive working relationships with residents, families, providers, and coworkers. Maintain resident confidentiality and comply with HIPAA requirements. Support facility operations and assist other departments when appropriate. Demonstrate professionalism, compassion, and respect in all interactions. QualificationsRequired High school diploma or equivalent. Valid driver's license with an acceptable driving record. Ability to safely operate facility transportation vehicles. Strong communication, customer service, organizational, and time-management skills. Ability to work independently and prioritize multiple tasks. Preferred Certified Nursing Assistant (CNA) certification preferred but not required. Previous experience in long-term care, skilled nursing, transportation, healthcare scheduling, or customer service. Knowledge of medical terminology and appointment scheduling processes. Physical Requirements Ability to assist residents with mobility needs. Ability to lift, push, pull, and transfer items or equipment as necessary. Frequent walking, standing, bending, reaching, and sitting. Ability to safely assist residents entering and exiting vehicles.
