MOBILITY TRANSPORT SPECIALIST JOB SUMMARY
After notification by facility staff of needed appointment, will schedule all appointments for residents/patients/clients, as well as for other individuals who may request transportation external to Liberty Health facilities. Utilize transportation management software to schedule, assign, and dispatch trips in real time. Monitor live vehicle locations and driver status to ensure on-time performance. Optimize routes to maximize efficiency, reduce mileage, and control costs Adjust schedules dynamically to accommodate cancellations, urgent add-ons, and delays. Serve as primary point of contact for drivers during active transports. Communicate schedule changes promptly to facilities, families, and medical providers. Will maintain a calendar of all appointments so that none will conflict. Will follow all safety guidelines to ensure resident transports are performed in the safest possible manner. Will maintain required logs of each transport and for each vehicle. Completes Quality Assurance monitors according to policies and procedures. Will establish and maintain a maintenance schedule for each vehicle that is used in transports and assist with basic vehicle maintenance including inspections, restocking, vehicle and equipment cleaning/disinfecting . Assists in maintaining a good working relationship with the community, hospital, physician offices and other health care facilities and agencies and assuring coordination of services for residents/clients. Maintain accurate trip data within transportation software system. Generate and review daily, weekly, and monthly transportation reports. Track key performance indicators (KPIs) such as on-time performance, trip volume, cancellations, and driver productivity. Assist in onboarding and training drivers on dispatch procedures and software use. Ensure accurate documentation for billing and compliance purposes. Provide daily route briefings and updates. Troubleshoot software scheduling conflicts and system errors. Coordinating all aspects of transportation is the primary duty of this position and will take precedence over these duties, but understanding that transports fluctuate daily, these other duties will be the responsibility of this position when not involved in scheduling and coordinating transportation: Safely transporting residents/patients/clients Perform other duties as assigned by immediate supervisor
JOB REQUIREMENTS
Must be at least 21 years of age or older, must be of good moral character, and have a genuine concern for the elderly and disabled. Must have CPR, First Aid Training, and wheelchair safety training. Must possess one of the following certifications: Certified Nursing Assistant, Emergency Medical Technician, Emergency Medical Responder. Must have skill checklist completed by Liberty trainer prior to transporting residents. Must have a valid state Driver's License to operate a motor vehicle with an appropriate driving record. Criminal/Driving records must be approved by Transportation Manager. Must be willing to work with various department directors, patients, family members, and community health care providers. Must have the ability to plan, organize, and follow-up on work assignments. Must have the ability to make independent decisions and work well under pressure.
