Field Supervisor AmeriBest Home Care, LLC Philadelphia, PA Job Details Full-time 13 hours ago Qualifications Mid-level 1 year Communication skills Full Job Description The Field Supervisor is responsible for planning, coordinating, and documenting non-skilled supervisory visits for the Philadelphia and surrounding county areas of clients following Home Care policies and procedures. Field Supervisors assure compliance with local, state, and federal laws, Medicare regulations, and established company policies and procedures. The field supervisor is responsible for the direct supervision of the personal care assistant (PCA). The field supervisor performs field evaluations and serves as a mentor and support for field staff .
ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Assesses new client needs upon admission and develops the initial care plan. 2. Confirms schedule for care and follows up with clients to ensure that quality care is received. 3. Performs in-person/virtual client reassessments and notifies Team Lead of any changes in the client's condition that may necessitate changes to the established care plan. 4. Performs direct observation of PCAs for annual performance evaluation and provisional monitoring of newly hired staff. 5. Assists in closing client care gaps and conducts unannounced home visits as necessary. 6. Communicates client problems, concerns, changes, and complaints to appropriate staff, and prepares reports of incidents that could result in legal action. 7. Maintains and develops positive relationships with existing and prospective clients, demonstrating excellent customer service and setting an example for other staff. 8. Maintains daily schedule in HHAX and accurately completes visits in a timely manner. 9. Participates in staff meetings, agency sponsored in-services, and team meetings, as directed. 10. Performs other duties and assists with special projects as requested.
REQUIRED SKILLS AND ABILITIES
Excellent active listening skills and a strong service orientation. Able to supervise staff and coach them to improve functioning and overall agency services. Patient demeanor with the ability to relate to, empathize with, and show compassion for a variety of personalities. Good interpersonal, written, and verbal communication skills. Ability to work as part of a team and maintain positive working relationships.
EDUCATION AND EXPERIENCE
High School Diploma CNA or HHA certification preferred. Associate's degree or equivalent industry experience preferred At least one (1) year of previous experience in health care. Proficient with Microsoft Office Suite or related software. Must have the use of an automobile in good working condition, a valid driver's license, and automobile insurance compliant with company policies.
PHYSICAL REQUIREMENTS
Ability to move, transport, or position up to 50 pounds Ability to move or traverse about in offices and/or client homes, including ascending and descending stairs. Ability to communicate effectively and clearly with others to exchange information.
TRAVEL REQUIREMENTS A
valid driver's license and reliable personal vehicle are required for this position.
