Full Job Description Position Summary :
Community Supports Navigators(CSN) will assist individuals and families who are experiencing homelessness or are at risk of homelessness. The CSN will be responsible for outreach and engagement, helping individuals connect with the necessary resources and services to secure and maintain stable housing. Additionally, the CSN will assist Members in acquiring, retaining, and improving self-help, socialization, and adaptive skills necessary to reside successfully in the person's natural environment. City Net's programs always follow Housing First and Harm Reduction service models. City Net's programs and services are housing focused.
Work Location:
CSN are assigned a base office in Santa Barbara, and are dispatched daily throughout the region according to the contract needs.
Schedule :
4/10s (4 days a week, 10 hours each day): either Monday-Thursday or Tuesday-Friday, 8:00am-6:30pm Job Duties & Responsibilities Conduct outreach to individuals and families experiencing homelessness or at risk of homelessness in various locations including shelters, street outreach, community centers, and other public spaces. Build relationships with community organizations, landlords, and property managers to identify and secure housing opportunities for clients. Provide assessment and support to clients to identify their needs, strengths, and barriers to obtaining and maintaining stable housing. Assist clients in accessing and applying for affordable housing programs, rental subsidies, and other financial assistance as appropriate. Connect clients with social service agencies, healthcare providers, and other resources to address any physical or mental health issues, financial concerns, or other barriers to housing stability. Work collaboratively with other service providers that client is connected to ensure optimal service coordination. Facilitate and coordinate communication between clients and landlords, property managers, and other service providers to ensure a smooth transition into permanent housing. Develop and maintain client files and documentation according to agency and program guidelines including entry into HMIS and other databases. Participate in regular training and professional development opportunities to stay up to date on housing policies and best practices. Attend community meetings and events related to homelessness and housing to represent the agency and advocate for clients. Provide culturally appropriate health education information and outreach in community-based settings, such as homes, clinics, shelters, local businesses, and community centers for various Managed Care Plan (MCP) members. Transports clients in City Net vehicle as needed. Job Skills and Requirements A passion for helping vulnerable populations reach their potential, with a desire to make a difference. Bachelor's or Master's degree in social work, public administration, or a related field. Knowledge and understanding of regulations related to housing development and implementation of housing programs. Experience in providing training and consultation to staff and external stakeholders. Familiarity with the CalAIM program and its requirements. Strong analytical, problem-solving, and communication skills. Ability to work independently and collaboratively with diverse stakeholders. Proficiency in using HMIS software and other relevant technology tools. Excellent organizational skills and ability to manage multiple tasks and deadlines. Strong interpersonal skills and ability to establish rapport with service providers and other stakeholders. Ability to travel locally to attend meetings, trainings, and conferences as needed. For insurance purposes, needs to be at least 21 years old, must have a valid drivers' license, and be approved to drive a City Net vehicle by our insurance company. Employees driving their own personal vehicle for work purposes will also need to show valid liability insurance. Displays patience and self-control with employees, clients, customers, and the public-at-large. Working knowledge of the Microsoft Windows OS, Microsoft Office 365, and the Internet.
