Job Description:
Position Summary Under the leadership of the department director, the inpatient Heart Failure Program Manager is responsible for the development, implementation, and management of the heart failure program across the continuum of care using a multidisciplinary approach to patient centered care. The role ensures evidence-based, high-quality, and cost-effective care for heart failure patients while improving clinical outcomes, reducing readmissions, and supporting organizational goals related to quality metrics, regulatory compliance, and patient experience. Key responsibilities include program development and oversight, clinical quality and outcomes,care coordination, data and reporting, education and training, regulatory, community outreach and growth. Skills and competencies include strong leadership and interdisciplinary collaboration skills,knowledge of heart failure clinical guidelines and disease management, knowledge of quality improvement methodologies (e.g., Lean, Six Sigma), data analysis and improvement expertise,excellent communication, organizational, and project management skills, ability to drive change and influence clinical practice, strong interpersonal skills, and ability to manage direct reports. Experience 3-5 years of cardiovascular or heart failure clinical experience required Prior program management, quality improvement, or leadership experience preferred Experience with data analysis and performance metrics strongly preferred Team/workgroup participation experience preferred Demonstrated leadership, problem-solving, and critical thinking skills Familiarity with regulatory standards preferred Education Bachelor's degree required Master's degree (MSN, MHA, or related) preferred Licensure/Certifications Current California RN license Heart Failure certification (e.g. CHFN) within 6 months of hire Basic Life Support (BLS) required Advanced Cardiac Life Support (ACLS) preferred Equal Opportunity Employer #LI-AC1
