We are seeking an enthusiastic, reliable, and motivated Support Services Coordinator to join our team. The Support Services Coordinator provides direct support, coordination, and skill-building services to individuals and families across multiple service areas. This role focuses on promoting independence, improving quality of life, and ensuring individuals have access to the services and resources they need. The Coordinator works in both home and community settings to support individuals in achieving their goals through person-centered planning, service coordination, and hands-on support. This position is not limited to a single program and may support services across various areas based on participant needs and agency priorities. The position requires about 65% travel within the San Diego area.Responsibilities:
Direct Support & Skill Development Provide hands-on support to individuals in home and community settings Assist individuals in developing daily living skills, including: Household management Community access Communication and self-advocacy Support individuals in working toward their individualized goals Assessment & Service Planning Conduct assessments to identify individual needs and areas of support Develop and implement person-centered service plans Monitor progress and adjust supports as needed Service Coordination & Resource Navigation Coordinate services and connect individuals to: Community resources Healthcare providers Benefit systems (SSI, IHSS, Medi-Cal, etc.) Support scheduling and follow-through with appointments and services Collaborate with families, service coordinators, and providers Crisis Management & Problem Solving Provide support during crisis situations and help stabilize services Identify barriers and develop solutions to maintain continuity of care Advocacy & Participant Support Advocate for individuals' rights and access to services Support informed decision-making and independence Promote community integration and participation Documentation & Compliance Maintain accurate, timely, and detailed documentation, including: Case notes Service plans Progress updates Ensure compliance with: Title 17 regulations Agency standards Person-centered practices Collaboration & Team Engagement Participate in team meetings and case conferences Work collaboratively with Program Managers, Team Leads, and other staff Communicate updates and concerns proactively Significant local travel to meet with clients and coordinate services.Requirements:
- Strong communication, organizational, and interpersonal skills.
- Ability to work effectively both independently and as a team member.
- Proficiency in both Spanish and English required.
- Valid driver's license and access to a reliable and insured vehicle.
- Experience with Google Suite
- Bachelor's degree in social work, psychology, or a related field.
