Videos Duties Help The Compact Act Coordinor responsibilities include coordining transitions in care for Veterans receiving acute suicide care under COMPACT Act between varying venues of care, types of care (medical and mental health) and locion of care (direct care and community care) in collaborion with assigned care coordinors each step of the Veteran journey. The Social Worker Program Coordinor instrumental in providing connection and referral to assist with proper evaluion for eligibility in other programs and is the facility subject mter expert on the COMPACT Act. Social Worker Program Coordinors in the GS-1 2 assignment provide complex care in a highly specialized, complex area and may be the sole practitioner in the program. They are responsible for daily operions of the program area and are responsible for, or contribute to, all aspects of clinical and administrive programming. GS-12 Social Worker (Program Coordinor) (1) Experience and Educion . One year of experience equivalent to the GS-11 grade level, which is defined a s: Working knowledge of community resources, how to make approprie referrals to community and other governmental agencies for services, and ability to coordine services; Independently conducting psychosocial assessments and trement interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educional and other diversified backgrounds; Working knowledge of medical and mental health diagnoses, disabilities and trement procedures (i.e. acute, chronic and traumic illnesses/injuries, common medicions and their effects/side effects, and medical terminology) to formule a trement plan; Independently implementing different trement modalities in working with individuals, families, and groups who are experiencing a variety of psychiric, medical, and social problems to achieve trement goals; Providing consultion services to new social workers, social work gradue students, and other staff about the psychosocial needs of pients and the impact of psychosocial problems on health care and compliance with trement. Experience must demonstre possession of advanced practice skills and judgment, demonstring progressively more professional competency. Program coordinors the GS 12 grade level are administrively responsible for a clinical program providing trement to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliive Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinor may be the sole practitioner in this specialty the facility and typically provide direct pient care services in the program area. The program coordinor oversees the daily operion of the program, develop policies and procedures for program operion, and prepare reports and stistics for facility, VISN, and nional use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documention, workload, da entry, ethical practice and service delivery. The program coordinor provides analysis and evaluion of clinical program da and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, opering within th budget, and accounting for appropried funds. The program coordinor is administrively responsible for the clinical programming and prepares reports and stistics for facility, VISN, and nional use. They provide leadership, direction, oriention, coaching, in-service training, staff development, and continuing educion programs for assigned staff. They initie and conduct a variety of program or service audits and complete designed clinical practice audits and reports, including productivity assessments.
