This role is responsible for coordinating engaging recreational, social, and wellness activities tailored to older adults while collaborating closely with the interdisciplinary care team. The ideal candidate is compassionate, organized, culturally sensitive, and passionate about improving quality of life for seniors. •This position is full-time and direct hire•
Schedule:
Monday - Friday from 8am - 5pm Responsibilities Coordinate and lead recreational and social activities for older adult participants Support individualized care plans alongside the interdisciplinary care team Monitor participant wellbeing and communicate updates to the care team Document participant progress in Epic EMR Organize group activities, outings, holiday events, and wellness programming Help create a safe, welcoming, and engaging day center environment Collaborate with community partners, volunteers, and therapy staff Qualifications Bachelor's Degree in Recreational Therapy, Psychology, Social Work, Human Services, or related field 2+ years of experience in social, recreational, healthcare, mental health, or activities programming Experience working with older adults preferred Current BLS & First Aid Certification Cantonese or Mandarin or Vietnamese fluency required Comfortable using Epic EMR Benefits Free Medical, Dental & Vision Insurance 401(k) with 8%
