We are seeking a dedicated Dementia Programs Manager to lead the design, implementation, and management of our dementia management programs. This role is a key connector between internal teams and external partners, ensuring programs are operationally efficient, compliant, and deliver high-quality care. What You'll Do As the Dementia Programs Manager, you will: Partnership Development & Management Build and maintain strong relationships with internal teams and external clinical/home care partners. Develop playbooks and onboarding approaches for new partners and programs (GUIDE and MA patients). Ensure CMS portal accuracy for patients and providers, and maintain key datasets and tools. Create training materials, webinars, and resources to support partner success. Cross-Functional Collaboration Partner with Clinical, Growth, and Product teams to optimize program design and implementation. Support internal teams with SOPs, job aids, and process improvements. Identify opportunities to enhance patient, partner, and care team experience. CMS Partner Requirements Management Collaborate with Care Navigators to maintain accurate partner rosters and CMS data. Communicate with CMMI and respond to inquiries. Track program success, collect patient stories, and report outcomes. Other Responsibilities Support newsletters, reporting, and analytics to enhance team performance. Take on other duties as assigned to support the growth and success of our programs.
What We're Looking For Education:
Bachelor's degree in a related field (or equivalent experience).
Experience:
3-5 years of progressively responsible experience managing people, projects, or programs. Strong communication, relationship-building, and problem-solving skills. Experience in clinical program management, dementia care, or related healthcare fields is a plus, particularly experience supporting programs aligned with the GUIDE Model (Guiding an Improved Dementia Experience) or other value-based dementia care initiatives.
