This includes Enhanced Care Management (ECM), CalAIM Community Supports (Housing Services and Transitional Rent), street outreach, housing navigation, and on-site supportive services for housed individuals. The Program Manager leads a team of supervisors to deliver person-centered, trauma-informed, harm reduction-based care coordination and housing services to individuals experiencing homelessness, complex health conditions, and multiple system involvement. Key Responsibilities Program Leadership & Integration Provide strategic direction across all programs — ECM, Community Supports, street outreach, housing navigation, and housing site supportive services — ensuring coordinated, integrated service delivery. Develop and maintain policies, procedures, and workflows that support cross-program collaboration and efficiency. Serve as primary point of escalation for complex cases requiring cross-program coordination or senior leadership decision-making. Supervision & Staff Development Directly supervise ECM Supervisor, Community Supports Supervisor, and Outreach Supervisor through regular individual and group supervision. Build supervisor capacity in staff oversight, performance management, coaching, and team leadership. Foster a culture of continuous learning, cultural humility, trauma-informed practice, and evidence-based service delivery. Compliance, Quality Assurance & Data Ensure compliance with DHCS CalAIM policies (ECM and Community Supports Policy Guides), outreach contract requirements, and housing program standards. Oversee case file reviews, documentation audits, and service delivery monitoring across all programs. Develop and implement corrective action plans; prepare for and participate in audits, monitoring visits, and contract compliance reviews. Oversee data quality and timely documentation across EHR, HMIS, and care management platforms. Budget & Resource Management Manage program budgets across all contract areas, ensuring fiscal responsibility and contract compliance. Monitor expenditures, forecast resource needs, and recommend budget adjustments or reallocation as needed. Oversee staffing levels, caseload ratios, and productivity standards to ensure capacity and prevent staff burnout. Partnership & Community Relations Represent City Net in community coalitions, collaborative meetings, and regional planning bodies addressing homelessness, healthcare, and social determinants of health. Facilitate effective referral pathways and warm hand-offs between programs and external service providers including Medi-Cal managed care plans, health systems, and housing agencies. Serve as a regional leader in identifying system barriers and collaborating with partners to improve access to housing, healthcare, and supportive services. Required Qualifications Minimum 2 years of supervisory or program management experience overseeing multiple programs or supervisory staff in care management, case management, or social services. Demonstrated experience serving individuals experiencing homelessness, complex health conditions, and/or behavioral health needs. Knowledge of California's CalAIM initiative, Medi-Cal managed care, ECM, and Community Supports programs.
Familiarity with housing systems:
HUD programs, permanent supportive housing, rapid rehousing, landlord engagement, and tenant rights. Experience in street outreach program management or on-site supportive services at a housing property. Strong written and verbal communication skills; able to present to diverse audiences including funders, partners, and community stakeholders. Proficiency with EHR, HMIS, and care management software platforms. Valid driver's license, reliable transportation, and ability to pass background check and meet provider credentialing requirements. Commitment to person-centered care, cultural humility, trauma-informed practice, harm reduction, and Housing First principles. Strong analytical skills with experience in data analysis, quality improvement, and performance management. Our Organization:
