Job Summary The Residential Service Coordinator provides coordination, monitoring, oversight, documentation review, and support for individuals receiving residential services through More Maitri's Host Home and Independent Living/IRSS programs. This position ensures compliance with HCPF, HCBS, and agency requirements while promoting person-centered services, health and safety, community integration, and quality outcomes for individuals served. The Residential Service Coordinator serves as a primary liaison between individuals receiving services, residential providers, families, guardians, case managers, medical professionals, and agency leadership. The position requires strong organizational skills, attention to detail, problem-solving abilities, and the capacity to work independently while maintaining high standards of service delivery and regulatory compliance.
Responsibilities Monitoring & Oversight:
Complete scheduled monitoring visits for Host Home and Independent Living settings; assess health, safety, rights, and service quality; identify and report concerns; maintain communication with individuals, providers, families, and support teams.
Documentation & Compliance:
Review and track service documentation, ISPs, MARs, incident reports, and other required records; follow up on gaps; assist with assessments, reports, and audits.
Individual & Team Support:
Coordinate medical, dental, and psychiatric appointments; support goal achievement; attend IDT and service plan meetings; connect individuals to community resources; promote person-centered, independent living.
Health & Safety:
Monitor appointments and health changes; assist with medication oversight; support emergency preparedness and drills.
Provider Support:
Coach and guide Host Home providers and residential staff; assist with onboarding and training; monitor policy compliance.
Cross-Coverage & Crisis Support:
Cover caseloads during shortages or emergencies; provide direct support and transportation as needed.
Administrative:
Attend meetings and trainings; maintain confidentiality; manage supplies and deadlines; complete assigned reports.
Qualifications:
Bachelor's in Human Services or related field preferred; 2+ years IDD experience; knowledge of HCBS/Colorado regulations; strong communication, organization, and documentation skills; valid driver's license; CPR/First Aid; Medication Administration certification.
Physical/Work:
Travel between sites required; may include evenings, weekends, or on-call; exposure to crisis situations possible.
Pay:
$23.00 per hour
