Responsibilities:

Coordinate the development, implementation, and maintenance of the Minimum Data Set (MDS) for all residents in the facility Manage and analyze data to ensure accurate and complete documentation of resident assessments, care plans, and services provided Collaborate with healthcare professionals, including physicians, nurses, and therapists, to ensure comprehensive care plans are developed and implemented Monitor and track resident progress, identifying areas for improvement and implementing changes as necessary Maintain accurate and up-to-date records and reports, adhering to state and federal regulations Participate in quality improvement initiatives to enhance the overall quality of care and services provided
