Job Description

Help Me Grow Program Manager The Help Me Grow Program Manager will be responsible to help lead and develop an internal team to support the following areas: Child Screening, Care Coordination, Program-specific Training, Community Events, Research, Data Tracking, and conducting Child Health Care Provider Outreach to encourage child health providers and community partners to refer families to 2-1-1, as appropriate, for more information about Help Me Grow Florida (HMGF) and other services and supports.
ESSENTIAL JOB FUNCTIONS
  • Oversee and sustain the Help Me Grow Lake system that will include 4 required core components: 1.
Centralized Access Point 2. Child Health Provider Outreach 3. Community & Family Outreach 4. Data Collection & Analysis Conduct Child Healthcare Provider Outreach (CHCPO) activities to educate and motivate child healthcare providers to conduct child screenings and to utilize the HMGF centralized telephone access points/make referrals directly to HMGL Team Engaging and utilizing a Physician Champion to educate and motivate physician and other child healthcare providers to understand the importance of conducting child development screening. Provide support to families in the community by answering child development questions. Educate families in the community on available screening tools to include Ages and Stages Questionnaire 3 (ASQ 3),

SE-2), Pediatric Symptoms Checklist (PSC) and the Modified Checklist for Autism in Toddlers (M-CHAT-R/F).
  • Establish a system with the Early Learning Coalition of Lake County to offer feedback to physicians and other child health care providers who have referred clients, including screening results, referrals given, outcomes, and appreciation.
Collaboratively maintain/update and make additions to the database of child development resources and referrals as needed. Works effectively as a team member and provides assistance to other staff members as needed. Build collaboration across sectors, to include early care and education, healthcare and family support. Assists in identifying gaps and barriers to services, and system issue trends families experience in utilizing services. Submit monthly data reports to Chief of Finance. Submit data reports for Plan and Conduct Semi Annual and Annual HMGL Leadership Meeting. Assimilate and submit training documentation, as requested such as agendas, certificates sign in sheets and training logs. Attends events and networking opportunities to develop and maintain a Help Me Grow presence across the county that will enhance public awareness, increase referrals, promote the benefits of Help Me Grow, and expand service opportunities. Collaborate amongst team on Family and Community Outreach with activities that develop associates and partners in the local community, promoting HMGF, and the importance of child developmental screenings across the community.
  • Develop, strengthen, and coordinate collaborative relationships with appropriate organizations and individuals, including the partnership with 2-1-1, including participate in various community meetings and outreach activities as determined appropriate by your supervisor.
Provide support for coalition and HMGF events and activities, as directed. Assist in identifying potential grants for sustainability.
Supervise Care Coordinator:
evaluation, time sheet, mileage Provide training to staff in HMGF initiatives and system as directed. Conduct/Attend Vision & Hearing Screenings, as well as other child focused events such as playgroups, and Books, Balls and Blocks events. Contribute to a harmonious working environment and support staff in the promotion of ELCLC strategies and goals.
  • Ensure that all information of confidential nature is held in a secure and confidential manner.
  • Attend, and assist with, a minimum of three community outreach events annually outside regularly scheduled operating hours of the Coalition, including evenings and weekends.
  • Attend a minimum of two professional development trainings annually as approved by your department officer.
Perform other duties as assigned within the scope of the position.
OTHER JOB FUNCTIONS
  • Attendance and participation in professional conferences, training, statewide and regional meetings, and other related events as it pertains to the position.
  • Work harmoniously with early education providers, ELCLC board members, school readiness agencies, and ELCLC staff.
  • Ability to maintain confidentiality with highly sensitive information.
  • Attends work related staff meetings and workshops as directed.
  • Participates in ELCLC collaborative efforts as needed.
  • Follows the ELCLC policy and procedure on reporting abuse and/or neglect.
  • Promotes and follows safety policies in the workplace.
  • Valid driver's license, appropriate insurance and reliable transportation.
  • Must submit to pre-employment screening including background check and meet minimum requirement for employment.
QUALIFICATIONS
  • Knowledge of community resources including systems, organizations and programs that deal with children and families facing developmental, behavioral and learning challenges.
  • Requires excellent interpersonal skills and the ability to work effectively with people of diverse backgrounds as well as interact with all levels of management.
  • Must have strong computer skills, including proficiency with Microsoft Office Suite. Knowledge of Statewide Information System preferred.
  • Well-developed communication skills, both written and oral.
  • Should be creative, energetic, a self-starter, able to prioritize and work independently with minimal supervision.
EDUCATION AND EXPERIENCE
Bachelor's Degree from four-year College or university; or one to two years related experience and/or training; or equivalent combination of education and experience. TRAVEL May be required to use his/her personal vehicle for out-of-office travel, and is reimbursed for such use at the prevailing rate. Proof of current auto insurance required.
BACKGROUND CHECK A
local and state FDLE background check will be completed. ELCLC is a drug-free workplace. Drug screening may be required prior to employment.
WORK SCHEDULE 40
hours per work week. Days and times may vary dependent upon the needs of the program.

PHYSICAL REQUIREMENTS
Must be able to lift up to 25lbs in order to meet the day to day requirements of the job. Must be able to sit for long periods of time with occasional times of standing and/or walking.

From $56,900.00 per year
