The individual holding this position is required to support and demonstrate their commitment to Longevity Health's purpose: To optimize the quality of life of people in long-term care by focusing on their individualized health needs. The Licensed Account Manager is a field-based role reporting to the Market Vice President and serves as the primary liaison between Longevity Health Plan and assigned skilled nursing facilities. This position is responsible for supporting care delivery, strengthening facility partnerships, and ensuring high-quality, patient-centered outcomes for members. This role combines account management, operational coordination, and sales responsibilities. The Account Manager will drive enrollment growth, manage facility relationships, and collaborate with cross-functional teams (clinical, network, and operations) to achieve performance, quality, and compliance targets. Key Responsibilities Account & Facility Management Serve as the primary point of contact for assigned SNFs Build and maintain strong relationships with facility leadership and staff Coordinate all plan-related functions and ensure effective implementation of I-SNP programs Maintain a regular visit schedule (minimum monthly onsite visits per facility) Manage SNF contractual relationships and ensure compliance with contract terms Identify and resolve operational issues and implement solutions Clinical & Operational Support Collaborate with internal clinical, network, and operational teams to support care delivery Assist with credentialing, claims resolution, audits, and data/reporting requests (e.g., HEDIS) Ensure accurate and complete member records across systems Lead and participate in multidisciplinary meetings with providers and facility teams Present data insights, education, and best practices to SNF leadership Program Implementation & Strategy Execute facility-specific strategies and enrollment goals in partnership with Market VP Lead onboarding and implementation of I-SNP providers and facilities Ensure facilities are fully prepared for program launch and ongoing compliance Track engagement, identify barriers, and implement action plans to improve performance Sales & Enrollment Growth Achieve monthly enrollment targets within assigned territory Develop referral networks and generate leads through relationship-building and facility engagement Conduct compliant sales presentations to residents, families, and responsible parties Manage lead tracking, application processing, and documentation Partner with Regional Sales Manager on strategy, training, and performance tracking Compliance & Reporting Ensure adherence to CMS regulations and company compliance standards Support audits, corrective action plans, and reporting requirements Maintain detailed documentation of activities, engagements, and outcomes Participate in internal meetings, including sales, clinical, and operational calls Licensure & Certifications Health Insurance Producer License (Medicare) required or ability to obtain within 60 days of hire Valid driver's license with a clean driving record Proof of automobile insurance meeting minimum coverage requirements Education Bachelor's degree required Experience Minimum 3+ years of healthcare industry experience (provider relations, contracting, or business development preferred) At least 2 years of sales experience with demonstrated success meeting targets Experience working with Skilled Nursing Facilities or long-term care environments preferred Skills & Competencies Strong communication and presentation skills across all organizational levels Ability to manage multiple priorities and projects in a fast-paced, field-based environment Solid understanding of managed care, healthcare contracting, and provider networks Strong problem-solving, analytical, and relationship-building skills Proficiency in Microsoft Office Suite and general business systems Additional Requirements Comfortable working onsite in Skilled Nursing Facility settings Ability to travel regularly within assigned territory, with occasional travel to other markets
