Role:
Unlimited Abilities is seeking a compassionate and organized Service Coordinator to support adults receiving Home and Community-Based Services (HCBS) and Habilitation (HAB) services. This role serves as the primary point of contact for members, families, and stakeholders while ensuring compliance with waiver regulations and delivering high-quality, person-centered support.
Key Responsibilities:
Coordinate client admissions, assessments, intakes, and onboarding. Develop, update, and maintain Individual Service Plans (ISPs) and client records. Conduct monthly face-to-face visits with individuals receiving services. Collaborate with House Leads, care teams, and community partners to ensure services are effectively implemented. Assist with referrals to community resources and support services. Review and follow up on incident reports and quality assurance recommendations. Maintain accurate documentation and ensure compliance with all state, federal, and agency requirements. Serve as a liaison between clients, families, providers, and other stakeholders. Participate in meetings, trainings, and ongoing quality improvement efforts.
Qualifications:
Strong organizational and communication skills. Ability to manage multiple priorities in a fast-paced environment. Experience in human services, case management, social work, healthcare, or a related field preferred. Commitment to person-centered care and supporting individuals in achieving their goals.
