Key Responsibilities:

Develop and oversee individualized service plans (ISPs) that align with person-centered goals. Working alongside staff and individuals needing support Ensure compliance with all state and federal regulations. Maintain thorough and accurate documentation, reports, and assessments. Serve as a liaison between individuals, families, and interdisciplinary teams. Organize and participate in outreach initiatives and networking events. Advocate for individuals to enhance their quality of life and community involvement. Work both independently and collaboratively to ensure the highest standard of care.

Qualifications:

Bachelor's degree in human services, social work, psychology, or a related field (required). Minimum of one year of experience working with individuals with intellectual and developmental disabilities. Experience in community engagement, or outreach (preferred). Exceptional documentation and organizational skills. Strong communication and interpersonal abilities. Self-motivated with the ability to thrive in a fast-paced, team-oriented environment . Outgoing personality with a passion for advocacy and service.
