We are seeking an experienced and compassionate Program Manager/QDDP to join our leadership team and oversee the delivery of Home and Community-Based Services (HCBS) throughout Indiana. Position Summary The Program Manager/QDDP is responsible for overseeing and coordinating services for individuals receiving Home and Community-Based Services (HCBS) under Indiana Medicaid Waiver programs. This role ensures compliance with Bureau of Developmental Disabilities Services (BDDS), Division of Disability and Rehabilitative Services (DDRS), Medicaid Waiver requirements, and agency policies while promoting person-centered supports and quality outcomes. The ideal candidate is a strong leader with experience supporting individuals with intellectual and developmental disabilities and a passion for helping people achieve their personal goals and independence. Key ResponsibilitiesService Planning & Coordination
- Develop, implement, and monitor Individualized Support Plans (ISPs) in collaboration with individuals served, families, guardians, case managers, and interdisciplinary team members.
- Ensure person-centered planning practices are followed and documented according to state and federal requirements.
- Monitor individual goals, outcomes, and progress, making recommendations for service adjustments as needed.
- Maintain accurate and timely service documentation. Leadership & Staff Supervision
- Provide supervision, guidance, and support to Direct Support Professionals (DSPs) and program staff.
- Conduct staff training, coaching, and performance evaluations.
- Promote employee engagement, accountability, and professional growth.
- Assist with staffing coordination to ensure individuals receive appropriate supports. Compliance & Quality Assurance
- Ensure compliance with BDDS, DDRS, Medicaid Waiver, HCBS, and agency standards.
- Monitor service delivery to ensure quality, health, safety, and regulatory compliance.
- Complete and review required documentation, including incident reports, monitoring notes, monthly summaries, and other regulatory records.
- Participate in audits, state reviews, licensing reviews, and quality assurance activities. Advocacy & Community Integration
- Advocate for the rights, dignity, choices, and well-being of individuals receiving services.
- Support individuals in accessing community resources, employment opportunities, educational programs, and social activities.
- Build strong relationships with families, guardians, healthcare professionals, case managers, and community partners. Administrative Responsibilities
- Maintain service records and program data.
- Assist with policy implementation and program development initiatives.
- Support continuous quality improvement efforts throughout the organization.
- Participate in agency meetings and strategic planning activities. QualificationsRequired Qualifications
- Bachelor's Degree in Human Services, Social Work, Psychology, Sociology, Rehabilitation Services, Special Education, or a related field; OR
- High School Diploma/GED with a minimum of five (5) years of progressive experience in human services, developmental disabilities, residential services, case management, or a related field. Additional Requirements
- Minimum of one (1) year of direct experience working with individuals with intellectual and developmental disabilities (IDD).
- Knowledge of Indiana Medicaid Waiver programs, HCBS requirements, and person-centered planning.
- Strong leadership, communication, organizational, and problem-solving skills.
- Valid driver's license, reliable transportation, and willingness to travel as required.
- Ability to work flexible schedules when needed. Preferred Qualifications
- Previous experience as a Program Manager or QDDP in Indiana.
- Experience with BDDS, DDRS, and Medicaid Waiver compliance requirements.
- Knowledge of Therap, INsite, or similar electronic documentation systems.
