Summary:
The Clinical Program Coordinator Gateway oversees the administration and clinical operation of Gateway Arts, including all activities related to the daily operations of the program. The Clinical Program Coordinator is responsible for ISPs, clinical programs, and other treatment plans, licensing, quality and safety standards, community based programming, and contact with persons served and families.Job Duties and Responsibilities:
The essential job duties/responsibilities of the position include but are not limited to the information listed below: Conducts regular monthly staff meetings. Ensures staff is trained on all necessary plans, procedures, and practices. Coordinates/oversees consents and doctor orders for Individual recordsObtains Seizure Protocols:
coordinates doctor's orders and consents. Completes supportive and protective devices paperwork; doctors orders and monthly checks, reports to Vinfen Quality Dept. Performs EHR (Electronic Heath Record) data entry using OARS, HCSIS, and other data platforms. Medication updates ISP assessments and objectives Internal Self Audits Ensures compliance with all internal quality and external licensing, certification, and accreditation standards and regulations, as well as program specific policies and procedures. Staffing and leadership during community outings Aids with community integration onsite and offsite. Community outing data entry and documentation Oversee documentation of certification standards specifically in regards to Visiting Artists Ensures accurate, timely site-level reporting (ex. incidents and physical reports)Maintains Gateway's Individual Records and Progress Records:
Requests missing and annual documents. Enters record updates in database, updates annual forms throughout records. Prints new monthly Progress Data. Collects completed monthly progress data, monitors progress notes Writes Quarterly Progress Notes. Processes ISP paperwork. Supervises clinical tasks, works with staff to achieve completion of tasks before deadlines. Ensures compliance with all aspects of ISP process and develops as appropriate implements, reviews, and trains staff on clinical programs. Provides support to case managers, as needed, regarding documentation completion. Completes comprehensive assessment for persons served Cases manages a minimum of at least 4 persons served at Gateway Arts. Maintains open communication with Gateway Arts' Program Directors, case managers and service coordinators, service providers, and other relevant parties, as needed. Attends case conferences and other relevant meetings with funding sources and Vinfen ISP meetings Vinfen Sustainability mtgs/Site manager mtgs Provides outreach and creates environments in which family, friends, and established relationships of the person served can be fostered and supported. Serves as a resource for program staff, family, and friends of persons served, neighbors, public officials, and other providers. Monitors daily activities of persons served. May provide crisis intervention, and direct care, as needed. Opens and closes the facility, as scheduled or as needed. Escorts participants into and out of the facility, as needed, and assists with transitions. Assists with general clean-up. Assists with special projects as assigned Performs other related duties, as required.Qualifications:

A minimum of three years' experience in human services. Preferred /Required Education:
A Bachelor's degree in Human Services or related required. In some cases, experience may be substituted for academic training.Driving Requirements:
Driving is a requirement for this position using either a Vinfen van or personal vehicle. If using a personal vehicle, you must possess and maintain adequate insurance as well as maintain a safe driving record which is subject to annual checks. A valid driver's license must be presented at the time of employment. Incumbents must be at least 21 years of age, have maintained a valid US driver's license for at least one year, and must be able to pass a driver's screening background check.Physical Effort:
Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb as this is a very physically active position. Must be able to lift at least 25 pounds using proper lifting techniques or the use of a two-person lift. Ability to operate a computer and other office equipment such as a calculator, copier, and printer. Ability to sit, reach, climb stairs, and maneuver through narrow spaces or hallways. Ability to assist clients with tasks of daily living. Ability to remain in a stationary position 50% of the time as needed. Ability to bend, reach, file, sit, stand, and move around the facility. Ability to speak, hear, and communicate with clients, staff, and external representatives. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus. For positions in day programs or group residences, the ability to assist in routine living activities including cleaning, meal preparation, vacuuming, shoveling, and grocery shopping.Required Certifications:
