Aug 31, 2026 Job Description The Dighton-Rehoboth Regional School District is seeking a dedicated and compassionate MERITS Coordinator to lead our transition program for students ages 18-22 with moderate to severe disabilities. The coordinator will design and deliver individualized transition planning, establish vital community partnerships, and empower students with real-world skills needed for adult life.
Qualifications:
Bachelor's degree required; Master's degree preferred Massachusetts DESE certification in Moderate to Severe Disabilities Demonstrated commitment to working with young people with disabilities Experience working with students aged 18-22, including community-based instruction and job coaching Responsibilities Include (but not limited to): Develop and implement individualized Transitional Planning Forms and IEPs Oversee paraprofessionals, job coaches, and volunteers Conduct annual transition assessments and surveys Build and maintain community partnerships (e.g., businesses, libraries, schools) Coordinate job shadowing, internships, and volunteer placements Provide family support and training regarding transition services and eligibility Design individualized curriculum focused on:
  • Activities of Daily Living (ADLs)
  • Vocational and life skills
  • Postsecondary education and employment readiness Maintain accurate student records and submit required documentation Collaborate with the High School Leadership Team and attend required meetings Lead interagency meetings and complete 688 referrals as necessary
