$26.45 Overview The Program Coordinator manages the supported housing program(s) with the overall responsibility for service delivery to formerly homeless individuals with the ultimate objective of assisting clients with achieving independent living. The Program Coordinator provides case management and supportive counseling to the clients within two locations; advocates for the clients' needs and supports them to meet the goals on their Individualized Service Plans. The Program Coordinator also assumes responsibility for the facilities and for supervision and oversight of the House Managers. Essential Job Functions Using Motivational Interviewing and a collaborative approach, conduct in-depth assessments to best determine the immediate client concerns including, but not limited to: financial, legal, mental health, substance abuse , employment, health care and housekeeping; Provide comprehensive case management, meeting at least once a week with each resident; Works with residents to Develops individual service plans (ISP) which include educational, vocational, medical, life skills goals. Meet with residents in the program weekly to review ISP progress and plan next steps Available to the residents for counseling and crisis intervention, and makes referrals to community programs for follow-up and treatment for substance abuse mental health, income enhancement and housing Obtain resources and services as requested and/or needed to address barriers to independent living including but not limited to; CORI mitigation, credit repair, and assistance with financial applications Develop partnerships and collaborative relationships with internal and external service providers to ensure client receives comprehensive services; Assist client with move-in transition supports including but not limited to; furniture and furnishings assistance, budget support, and other resources and services as needed; Document all interactions with clients utilizing appropriate reporting mechanisms, including web-based data collection systems; Responsible for coordinating the houses programs and services; Provides direct supervision to house managers and ensure proper coverage of all shifts; Attends a weekly meeting with the Program Manager and trainings as required; Serve as liaison with Community based organizations, attends meetings with such organizations as needed; Facilitates residents' house meeting, arranges for guest speakers, and advocates for the residents at staff meetings; Completes reports for program as needed; Attends meetings with outside provider agencies; Performs other tasks as requested by the Program Manager, Director of Individual Services or the Sr. Director of Programs Observes all shelter and agency rules and policies.EEO STATEMENT

Cubicle space within office building; prolonged sitting at a desk and working on a computer, OR, requires equal amounts of sitting and standing, must be able to climb stairs. Position requires a minimum of four days onsite. Office is climate controlled with heat and air conditioning. Requirements BA Required A minimum of two years working with homeless individuals and/or homeless families and in human services required. Experience with addictions and substance abuse treatment is preferred. Supervision and team building experience required. Proficiency in Microsoft office software and knowledge of databases used to track outcomes for record-keeping. Second language a plus. Good boundaries and self-knowledge. Must be available to be on-call as needed Knowledge of community resources; Able to exhibit sound case management concepts and exercise good judgment; ability to manage multiple tasks; ability to work both individual and as a contributing member of a team; and, ability to relate to persons of diverse backgrounds and functioning levels.
