1 to 3 years (depending upon degree)•2 years working with homeless persons with disabilities Job Description Are you interested in working in the mental health field and working with adults who are homeless, CHCS currently has an opening for a Navigator on our Maine Projects for Assistance in Transition from Homelessness Team. Will work as a member of a team of professionals who provide outreach, engagement, case management and service coordination services to adults, who are homeless and have a mental illness or co-occurring mental illness and substance use disorder. These individuals are eligible under the Maine Projects for Assistance in Transition from Homelessness ( PATH ) Program. The PATH Navigator position will be responsible for the identification of eligible individuals, assessment of needs, planning, linking to resources, monitoring and evaluation of services and supports needed by homeless individuals. Must have the ability to participate in street outreach, engage people who are homeless, perhaps living outdoors, at shelters, or other community locations. Must have a valid State of Maine driver's license and automobile liability coverage of $100,000/$300,000. CHCS believes in the importance of work-life balance, we offer generous leave time with paid sick & vacation time plus seven paid holidays and five paid float holiday time to our full-time staff and pro-rated time for part-time staff.
