Oversee, execute, and verify comprehensive post-acute transition plans for patients transitioning from acute care settings.
Address and process requests for post-acute services from both inpatient clinicians and community-based providers in a timely manner.
Coordinate patient placements with skilled nursing facilities, rehabilitation centers, home health agencies, and other post-acute care providers.
Communicate and collaborate with insurance companies and payers to obtain necessary authorizations and verify coverage for post-acute services.
Maintain accurate and detailed documentation of all patient transitions, including clinical summaries, care plans, and placement confirmations.
Collaborate with interdisciplinary healthcare teams, including physicians, nurses, social workers, and case managers, to ensure seamless care coordination.
Follow up on patient outcomes and post-acute care quality to ensure appropriate placement and continuity of care.
Ensure all transitions comply with regulatory requirements, accreditation standards, and organizational policies.
Munson Healthcare requires all employees be vaccinated or have lab confirmed immunity for Measles, Mumps, Rubella and Varicella. MHC also requires all employees to receive a flu vaccine during the flu season in the year that they are hired and annually thereafter, or receive an approved medical or religious exemption.
