Key Responsibilities:

Work directly in the home and/or community with assigned client(s). Participate in Individual Support Plan meetings and RN visits. Train staff on each client's medical needs and personal preferences. Ensure staff are knowledgeable about client medications and potential side effects. Oversee daily residential notes, goals, and health tracking. Maintain checklists and schedules related to client care and appointments. Ensure safety and cleanliness of the living environment, including proper food storage. Collaborate with the Program Coordinator on follow-ups and staff training. Document and report any significant occurrences or concerns. Education, Experience, and/or

Credential Qualifications:

Must be 18 years of age; must have a high school diploma or GED. One year experience in the field of intellectual/developmental disabilities, or successful completion of training in the Missouri Quality Outcomes approved by the Division of DD regional office.

Additional Qualifications:

Completion of background checks including criminal record, driving record, abuse/neglect, and fingerprint checks. Current driver's license, acceptable driving record, and current auto insurance.

• Sedentary work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, or pull objects.

Repetitive movements of hands, fingers, and arms for typing and/or writing during the work shift.
