Resident Services Coordinator:
Education:
one of the following required - High School Diploma or equivalent with significant work experience Some college coursework Associate's degree 3 years of experience in services for the elderly, disadvantaged or social services in a health care or related setting. 5 years of experience in services for the elderly, knowledge of entitlements programs, health care and dementia issues preferred Responsibilities for a
Resident Services Coordinator:
Connect residents and home-based participants with supportive services to promote successful aging in place. Assist residents and families with transitions to higher levels of care, coordinating with outside agencies as needed. Develop and maintain a comprehensive community resource guide. Monitor and prioritize support for frail residents through regular contact and post-hospital/therapy follow-up. Track and report service outcomes, including monthly and annual reports as required. Develop and distribute communication materials (newsletter, website updates, social media, marketing content). Serve as liaison to resident groups, families, and community partners. Plan and coordinate social, educational, spiritual, and vocational programs to reduce isolation and support resident well-being. Maintain a monthly activity calendar and support development of congregate and HCBS programs.
