Salary:
$21.00- $22.00 hourly
Minimum Qualifications:
Associates Degree in Health/Human Services and two years experience preferred. Valid NYS Drivers' License. Combination of education and experience considered. Certification as a QHP or CRPA required.
Job Summary:
In accordance with the organization's mission, vision and values, the COTI Coordinator is responsible for the daily operation of those programs. The coordinator will assure agency & program compliance and fidelity in strategic business goals.
The following duties will apply:
Duties:
1. Facilitates motivational team building and group supervision. 2. Provides direct oversite to Peers and Clinicians on a regular basis by various audits and observations and coordinate appropriate supervision as/if needed. 3. Complete staff evaluations as prescribed by the agency. 4. Lead and participate in general, staff, in-service, and supervisory meetings held by the Agency. 5. Maintain required reporting, records and statistics as directed. 6. Develops and maintain positive and effective relationships with community groups, referral sources and service agencies. 7. Participate in community presentations and training programs. 8. Assess staff training needs and assist in the development of those needs. 9. Training of all new direct reports. 10. Assure coordination of care. 11. Maintain program functioning through appropriate staff scheduling 12. Contribute to the development and implementation of COTI and in-community policies and procedures 13.Ensures staff complete appropriate follow up with all referral sources to advise of referent status and to request and receive additional required information and/or documents. 14. Ensures all clients placed on the wait list receive communication per regulation. 15. Answers phones and triages for appropriate support either Tele Health or in Community Screens/evaluations or assessments, crisis calls, asses for safety, Bed filling, updating referrals approving screens. 16. Provides monthly statistical reporting and data for analysis of residential intake programming. 17. Will utilize a collaborative approach with all Residential leadership to ensure maintenance of bed utilization 18. Perform other duties and functions as directed. Other Skills/Knowledge and Experience 1. Demonstrated ability to communicate effectively and work cooperatively with culturally diverse persons, staff and community service providers. 2. Knowledge of and experience with the behavioral health system. 3. Proficient computer skills to generate reports and produce electronic records. 4. Ability to multi-task, have good problem solving & time management skills. 5. Excellent written and verbal communication skills. 6. E-record competency. 7. Familiarity/comfort with in-community as well as potential crisis services
