HIV/AIDS.
This includes conducting initial and ongoing comprehensive behavioral risk assessments, developing client-centered action plans, reviewing action plans, service documentation, and maintaining client files as appropriate. 4) Provide linkage support to promote engagement in essential and/or medical support services (ex: food pantry, housing assistance, insurance navigation, name change) 5) Provide rapid HIV, confirmatory HIV, and syphilis testing as needed. 6) Promote the initiation of, retention in and adherence to client-centered sexual health-based medical interventions. 7) Maintain all required electronic and written records of testing, community advisory board meetings/activities, linkages and outreach activities, in addition to maintaining monthly statistics of clients served. 8) Additional activities as assigned by theRegional Program Manager Requirements:
Master's degree in Social Work, Mental Health Counseling or related field preferred. Bachelor's degree considered with proven demonstration and knowledge of sexual health prevention work, program management, with supervisory experience (2 years minimum). Professional or lived experience working with or beingBIPOC/TPOC
preferred. Strong knowledge of HIV prevention, PEP/PrEP, U=U, sexual health education, and the LGBTQ+ community in Suffolk County preferred. Ideal candidate will be self-motivated, have strong organizational and communication skills, exhibit a high level of trustworthiness, and have strong interpersonal skills. Bilingual English/Spanish is a plus. Work schedule requires both evening and weekend availability. Knowledge of Microsoft Office 365 (Outlook, Excel, Word, Teams, Sharepoint). Travel throughout Nassau, and Suffolk Counties is required- must have own transportation (car and driver's license) or other available means to travel.
