PFY is seeking to hire a full-time Prevention Services Coordinator who will have the primary role of providing strengths-based case management services aimed at supporting engagement in sexual health and essential support services, supportive counseling services, and provide staff supervision and development to emerging LGBTQ+ youth leaders. To be successful in their role, the Prevention Services Coordinator, will have an understanding of the needs of LGBTQ+ young people of color and demonstrate the core values of the agency; respect, integrity, inclusiveness, collaboration, and dependability. As a member of our team you will become a part of a diverse, innovative, and passionate group of LGBTQ+ professionals. Your management team will be focused on helping you expand your skills, grow, and provide you with the needed support. All full-time staff have access to a robust paid time off package, 403B, diverse health insurance options, vision/dental, flexible health spending accounts, nonprofit loan forgiveness, flexible work schedules, and more. 1) Responsible for the supervision and professional development of two part-time LGBTQ+ peers and the initiative's full time Care Coordinator. This includes recruitment, providing weekly supervision, professional development, ongoing feedback and skill assessment, maintaining employee records, and ensuring programs/service deliveries are efficiently and effectively delivered. 2) Provide clinical backup to weekly/monthly programs/events as needed. 3) Provide client-centered, strength-based, case management services to LGBTQ+ individuals who are at-risk of HIV transmission or living with
HIV/AIDS.
This includes conducting initial and ongoing comprehensive behavioral risk assessments, developing client-centered action plans, reviewing action plans, service documentation, and maintaining client files as appropriate. 4) Provide linkage support to promote engagement in essential and/or medical support services (ex: food pantry, housing assistance, insurance navigation, name change) 5) Provide rapid HIV, confirmatory HIV, and syphilis testing as needed. 6) Promote the initiation of, retention in and adherence to client-centered sexual health-based medical interventions. 7) Maintain all required electronic and written records of testing, community advisory board meetings/activities, linkages and outreach activities, in addition to maintaining monthly statistics of clients served. 8) Additional activities as assigned by the
Regional Program Manager Requirements:
Master's degree in Social Work, Mental Health Counseling or related field preferred. Bachelor's degree considered with proven demonstration and knowledge of sexual health prevention work, program management, with supervisory experience (2 years minimum). Professional or lived experience working with or being
BIPOC/TPOC
preferred. Strong knowledge of HIV prevention, PEP/PrEP, U=U, sexual health education, and the LGBTQ+ community in Suffolk County preferred. Ideal candidate will be self-motivated, have strong organizational and communication skills, exhibit a high level of trustworthiness, and have strong interpersonal skills. Bilingual English/Spanish is a plus. Work schedule requires both evening and weekend availability. Knowledge of Microsoft Office 365 (Outlook, Excel, Word, Teams, Sharepoint). Travel throughout Nassau, and Suffolk Counties is required
