About us The Children's Health and Research Foundation (CHRF) supports, promotes, and conducts research to improve the health and well-being of children and families in New York's Hudson Valley and surrounding regions. With a focus on the most vulnerable populations, CHRF aims to enhance health services for mothers and children while improving the quality of life for children diagnosed with chronic or serious medical conditions.

POSITION DESCRIPTION HEALTHY FAMILIES ROCKLAND COUNTY

(HFRC) Dual Role Home Visitor (Full-time position) Family Support Specialist/Family Resource Specialist Working together with its partners The Lower Hudson Valley Perinatal Network (LHVPN) advocates for and educates consumers and professionals about maternal, child and family health. LHVPN also mobilizes the community, to eliminate disparities, and improve the health of women, children and families in New York's Lower Hudson Valley. The LHVPN is a program of the Children of the Health and Research Foundation, INC.

Creole Position Summary:

The Healthy Families Rockland County dual-role Family Support Specialist (FSS) and Family Resource Specialist (FRS) supports expectant and new parents to get off to a great start on their parenting journey. This is achieved by conducting ongoing outreach, conducting regular assessments; engaging high-risk families through regular home visits, providing support to promote positive parent-child interaction and family/child health & development; and assisting families with goal planning to enhance family functioning, while making referrals, advocacy, follow-up, and overall service coordination with other providers. The position also requires working with a team to coordinate program activities to address family isolation.

Duties and Responsibilities:

Conduct community outreach, assessments, and home visits following model guidelines. Maintain a caseload of 20-25 families, performing weekly visits and daily outreach to potential partners and families. Collect referrals from community partners and keep an updated resource list for families. Ensure timely and accurate documentation in compliance with agency policies and program standards. Participate in required trainings, team meetings, and conferences, which may include overnight stays within New York State. Review participant files to ensure documentation aligns with agency policies and

standards. Assist in planning and executing group events to meet program goals. Engage in weekly reflective supervision sessions to support professional development. Adhere to

CHRF/LHVPN

policies, including time and attendance, mission, and code of conduct. Perform other duties as assigned in diverse settings, including homes, community environments, and the office.

Qualifications:

Associate's degree or higher in Human Services, Social Work, Early Childhood Education, or a related field. Experience working with high-risk or at-risk children and families in a community-based setting. Knowledge of home visitation programs, infant/child development, or family support services is preferred.

Skills:

Strong ability to engage, network, and present to individuals and groups. Organized with excellent verbal, written, and presentation communication skills. Culturally sensitive and committed to health equity and reducing health disparities. Proficient with computers, including word processing, spreadsheets, and databases. Self-motivated, solution-oriented, and effective working independently or collaboratively. Other Conditions of Employment A valid driver's license and reliable personal transportation is necessary to fulfill job requirements. Flexibility in travel in and out of Rockland County. Must have schedule flexibility to include occasional early morning, evening and weekend meetings/activities. Potential exposure to a variety of childhood and adult diseases and illnesses. Work in environments with heating and air conditioning, such as residential homes. State travel may be required for training and state meetings with overnight stay. Ability to meet the following physical requirements with or without reasonable accommodation: ° Walk, bend, kneel, and reach;° Lift 20 - 30 pounds;° Conduct both outdoor and indoor activities, in both warm and cold temperatures.
