Job Description

Job Summary The Program Coordinator for our Emergency Residential Services program plays a vital role in ensuring safe, stable, and trauma-informed care for youth in crisis placement. The ideal candidate brings experience in residential youth services, case coordination, or a related human services field, with a strong understanding of the unique needs of at-risk and transitional youth populations. Essential Duties & Responsibilities Ensure proper services are provided to address the wants and needs of people with developmental disabilities as written in the Individual Support Plan (ISP). Ensure a safe, positive, and encouraging atmosphere in the homes for the people we support. Ensure each individuals' rights are protected and they are treated with dignity and respect. Demonstrate and train team members to interact with the people we support in an emotionally supportive and therapeutic manner. Ensure all documentation is completed accurately and in a timely manner, including but not limited to individual plan data, behavior plan data, community integration logs, assessments, individual funds requests, communication book, mileage logs, maintenance requests forms, and supply acquisition forms. Safeguard individual's funds, assisting individuals with purchases. Ensure assigned team members record their time accurately and approve timecards in a timely manner. Ensures staff receive necessary training, both classroom and in-service. May conduct training when authorized. Performs regular and unannounced site visits. Reviews and responds to incident reports. Responds to requests from outside agencies in a timely manner. Facilitate the integration of individuals into the community by accompanying them to recreational and social activities. Intervene when aggressive and/or inappropriate behavior occurs to implement behavior plans and strategies. Provide on-call support to staff during emergencies and locate staff to fill shifts when necessary. Ensure assigned facilities and vehicles are clean and compliant with Company and state requirements. Complete and remain current on all required trainings. Other duties as assigned. Education & Experience At least two years of experience working with people with developmental disabilities preferred One year supervisory/leader experience preferred Valid driver's license Current automobile insurance Ability to pass registry and background checks Knowledge, Skills & Abilities A passion to serve others Strong attention to detail Excellent time management skills and the ability to juggle multiple responsibilities while communicating honestly about your timelines, challenges and questions. Ability to adapt as needed in a growing environment Ability to handle a variety of situations and personalities Must maintain confidentiality and protect individual rights Ability to always project a caring and professional image Excellent communication, interpersonal and organizational skills Physical Requirements Ability to be on-call and potentially provide coverage of assigned facilities Ability to communicate with other people in-person, by telephone, and in writing Ability to use verbal and physical intervention to redirect and/or restrain a person who is physically aggressive or self-abusive Ability to safely provide supervision and monitoring to meet the needs of people supported by Hands of Heartland Ability to perform repetitive tasks for extended periods of time to include but not limited to bending, stooping, sitting, and reaching Ability to lift up to 75 pounds as needed
