Program Specialist - Community Living Arrangements

As a Program Specialist

• CLA, you'll oversee residential sites and support staff while ensuring high-quality care for individuals in the Community Living Arrangement (CLA) program.

You'll play a key role in developing, implementing, and monitoring Individual Support Plans (ISPs), helping individuals achieve their personal goals and live as independently as possible.

What You'll Do:

Supervise and support residential staff, including training and performance oversight Develop, implement, and monitor Individual Support Plans (ISPs) Attend ISP meetings and ensure assessments, outcomes, and documentation are completed on time Conduct regular site visits and ensure quality care across all assigned homes Coordinate staff schedules, review time records, and assist with hiring Respond to on-call emergencies and complete required reports and investigations Ensure compliance with 6400 regulations and program policies Collaborate with families, support coordinators, and outside providers Support community-based activities and monitor individual progress Lead staff meetings, trainings, and ongoing development

What's Required of You:
