Job Description

The Housing Support Specialist provides intake and direct case management services to priority consumers who are in need of subsidized housing.
Job Duties Include:
Providing intake and authorization for consumers in the Continuum of Care (COC) program (COC). Sharing information and referral procedures with potential applicants and other community members regarding Shelter Plus Care programming. Managing grant funds that provide subsidized housing opportunities for persons with drug and alcohol and/or mental health concerns. Identifying landlords and properties that are appropriate for inclusion within the Shelter Plus Care Program housing options. Referring consumers to the other designated specialists for mental health assessment, and assisting consumers with accessing additional supportive services, as needed. Working with the Erie County Office of Drug & Alcohol to assist consumers in receiving any needed treatment. Providing direct case management services to priority consumers in the COC Program. Assisting consumers through the application process for subsidized housing through education and preparation of necessary documentation, as well as attendance at appointments. Supporting consumers through appeal processes, as appropriate. Developing strong working relationships with the City of Erie Housing Authority, the Erie County Housing Authority, and HANDS for advocacy on behalf of priority consumers. Documenting all contacts with or on behalf of clients in the clinical record, as required by department protocol.
Qualifications:
Bachelor's Degree and one year behavioral health experience or any equivalent combination of training and experience. Knowledge of Drug and Alcohol system and recovery community preferred. Must have valid driver's license and own transportation. FBI/Act 33/34 clearances required.
