Program Specialist/Caseworker 1.8 1.8 out of 5 stars 911 S Main St, Greensburg, PA 15601 $18.00 - $19.50 an hour - Full-time Westmoreland County Blind Association 10 reviews $18.00 - $19.50 an hour - Full-time Summary The Program Specialist/Caseworker is responsible for the operation and administration of the programs, services, and production activities relating to the employment of multi-handicapped and mentally challenged consumers of the WCBA. The Program Specialist/Caseworker provides necessary support for individuals with disabilities and visual impairment to accomplish necessary tasks related to health and independent living that would otherwise be impossible or impractical due to the extent of the individual's disability. Duties and Responsibilities Assist the Program Manager & Program Caseworker as needed Assuring that the agency complies with all waiver standards Responsible functioning Management/Risk Management Along with DOP, responsible for Quality Management Assure provider qualifications are valid Assure ongoing compliance with HCSIS, PROMISE and SSD maintenance is correct Attending Individual Service Plan Meetings Supervising, monitoring and evaluating services provided to the individual Maintain a safe and harassment free work environment Maintain compliance with ISO standards and procedures Maintain and monitor the regulations set forth in the Home Health Care Services including Personal Assistant and Respite Services Maintain continuing education and training requirements - minimum of 24 hours per year Transportation to medical doctors, hospitals, blood labs, grocery store and other social service agencies Chore services: assisting clients with shopping, reading mail, scheduling appointments
Recordkeeping:
daily, weekly, and monthly recording of services provided to clients and the time spent with individual clients All other duties as assigned by senior management Education/Experience Minimum of an Associate's degree or 60 credit hours in social work, social sciences or related field with at least 4 years' experience working with physically/intellectually challenged individuals Bachelors degree in Social Work, Human Services, or related field with at least 2 years' experience working with physically/intellectually challenged individuals Established safe driving record and valid state driver's license is necessaryPay:
