Then Person Directed Supports Inc. is place for you! PDSI provides supports and services for Individuals with Intellectual and developmental disabilities (IDD) as well as mental health diagnoses. Our Program Specialist position helps the Individuals live their lives to the fullest through person centered planning. Ideal candidates will possess a Bachelors degree with 2 years of IDD experience or an Associates degree with 4 years of IDD experience.
Job Responsibilities:
Creates and promotes a culture centered on building self-directed lives with the people we support. Displays principles of good management and applies these in daily work performance Serves as the liaison between PDSI and supports coordination agencies as well as county administrative entities Coordinates monthly meetings and emergency team meetings with outside supports coordination agencies Attends internal treatment team meetings and bi-weekly management meetings Tracks data on ISP outcomes, behavioral outbursts, community participation and health promotions Creates monthly and quarterly progress reports Produces annual assessments based on the individual's progress and growth Writes restrictive procedure plans for individuals with complex behavior concerns Makes independent decisions regarding program operations with his/her authority Notifies the Lead Program Specialist in an accurate and timely manner of unusual incidents, problems, and changes in individual Person Centered Plans Promotes a positive working relationship with all employees Coordinates and reviews the careful preparation with individual's records and ensures that they are kept accurate and up to date Provides direct support to individuals as needed Ensures that programmatic routines are followed as established; follows established procedures when making changes Remains aware of behavioral and /or medical status of the people that we support Records and reports to the Lead Program Specialist Participates in required in-service training and professional growth activities Provides counseling and assistance to individuals as needed Completes all other assigned tasks by Person Directed Supports, Inc.
Job Requirements:
