Primary Function The primary function of this position is to serve as an operational and coordination leader within the

This role bridges direct program management with system-level oversight, ensuring that both the Help Me Grow Centralized Access Point (CAP) and Identified Practice (IP) partnerships operate with fidelity to program goals. Help Me Grow is not a single program; it is a cross-sector network model that builds the local infrastructure needed to ensure every young child has timely access to developmental supports, early identification of concerns, and seamless connection to the services and resources their family needs. In Northeast Tennessee, this means partnering with pediatric and primary care providers, community organizations, and families across multiple counties to make sure no child falls through the cracks. This position will play a central role in bringing that network to life through building relationships, connecting families to resources, and strengthening the systems that set children up to thrive. Primary Responsibilities Practice Coordination Serve as primary point of contact for
NE TN GROWSS
(NETG) Identified Practices (IPs) and associated health care and community partners to integrated tele-behavioral health. Provide support through coaching, workflow shifts to increase fidelity, and relationship-building with providers. Support data collection, acquisition, and effective extraction. Collaborative Learning Sessions Schedule, coordinate, and manage all collaborative learning sessions and associated functions, including speaker outreach, participant communication, platform setup, and post-session follow-up. Collaborate with the ETN Director and program partners to develop session topics aligned with identified capacity-building needs and program goals. Maintain session calendars, materials, and documentation. Centralized Access Point (CAP) Oversight Oversee maintenance of an accurate, comprehensive, and user-friendly directory of local services and community partners within the CAP system. Ensure timely response to phone, email, and web-based referrals through the CAP. Follow up with families and/or referral sources to confirm connection to services, troubleshoot barriers, and ensure satisfaction with the process. Identify and communicate resource gaps to leadership to inform system-level improvement. Oversee documentation of all contacts, referrals, follow-ups, and outcomes in the Help Me Grow database, utilizing Findhelp and de-identified tracking sheets. Track referral status and monitor patterns in service utilization, unmet needs, and barriers to access. Generate regular reports for program evaluation, funder requirements, and stakeholder engagement & contribute to continuous quality improvement efforts to enhance CAP efficiency, responsiveness, and impact. Listen with empathy and ensure staff conduct thorough needs assessments to understand each family's strengths, concerns, and priorities. Collaboration & Community Outreach Build and maintain positive relationships with health, education, and social service providers to strengthen referral pathways. Participate in community outreach events, presentations, and training to increase awareness of Help Me Grow and the CAP. Support cross-sector collaboration efforts to improve early identification and linkage to services for young children and their families.
Required Skills and Abilities:
Excellent oral and written communication skills reflecting professional knowledge and demeanor in all forms of communication. Strong organizational skills, self-direction, and ability to manage multiple priorities simultaneously. Capacity to work effectively in an interdisciplinary team and engage families and partners from diverse backgrounds in a culturally responsive manner. Ability to maintain confidentiality and exercise sound professional judgment. Capability to collect data and engage in appropriate feedback.
Minimum Requirements:
Bachelor's in Health Care Administration, Public Health, Social Work, Early Childhood, Human Services, or related discipline required. At least 2 years of experience in healthcare, program implementation, care coordination, or related settings. Proficiency in Microsoft Office, including Word, Outlook, and Excel and ability to work in or learn programs related to creating, capturing, and evaluating data as needed. Experience working in a medical and/or clinical setting is preferred.
