Executive Director Job Summary:
The Supports Coordinator (SC) is responsible for providing person-centered service coordination to individuals with intellectual disabilities and/or autism. This includes developing, implementing, and monitoring Individual Support Plans (ISPs), connecting individuals to needed services and supports, and ensuring compliance with all regulatory requirements set forth by the Office of Developmental Programs (ODP) and Cypress Support LLC.
Responsibilities:
Develop and maintain ISPs in collaboration with individuals and their teams. Conduct regular monitoring visits in accordance with waiver guidelines. Coordinate and document all services and supports. Maintain accurate and timely records using organizational systems. Advocate for individual needs and ensure person-centered planning is followed. Attend required trainings and staff meetings. Comply with all ODP and agency policies and procedures.
Minimum Qualifications:
Candidates must meet one of the following education/experience requirements: A bachelor's degree with at least 12 college credits in a related social science (e.g., sociology, psychology, criminal justice). Two years as a County Social Service Aide 3 with two years of college coursework, including 12 relevant credits. Equivalent experience and training, including one year as a paraprofessional case manager and 12 relevant college credits. Two years of professional experience in developmental disabilities, special education, mental health, or related field, with at least 12 relevant credits. Must have experience as a Supports Coordinator, Program Specialist, or other role in the disability field of Pennsylvania.
Additional Requirements:
Valid driver's license. Valid Social Security number and authorization to work in the U.S. Criminal history clearance (renewed every 3 years). Child abuse clearance (renewed every 3 years). Completion of the ODP-required SC Orientation Curriculum.
