Program Coordinator (Part Time) (Home Care) 1100 Kermit Drive, Nashville, TN 37217 $20 an hour - Temporary, Part-time, Full-time $20 an hour - Temporary, Part-time, Full-time Program Coordinator (Part Time) VBP Health LLC is seeking a compassionate, organized, and dependable Program Coordinator to support individuals with developmental disabilities and oversee day-to-day program operations. The Program Coordinator will help ensure individuals receive quality support services while maintaining compliance with DDA, TennCare, and agency requirements. This role requires strong communication, problem-solving, and organizational skills, along with the ability to manage multiple responsibilities in a fast-paced environment. Key Responsibilities Coordinate services and supports for individuals receiving care through VBP Health programs. Assist with scheduling, staffing coordination, and coverage needs for individuals served. Maintain accurate and organized documentation in compliance with DDA, TennCare, and agency standards. Support survey readiness by ensuring required documentation, monthly checks, and compliance records are maintained. Communicate regularly with families, conservators, support coordinators, nurses, and community partners. Monitor individual needs and assist with appointments, follow-ups, and community-based services. Respond to urgent situations involving individuals or staff and assist with resolving operational concerns. Assist with incident reporting, documentation review, and follow-up actions when necessary. Help oversee staff performance, training compliance, and certification tracking. Conduct regular audits of documentation and program files to ensure compliance and quality assurance. Support onboarding and orientation of new staff members. Participate in care planning meetings and assist with implementation of person-centered supports. Promote individual rights, dignity, independence, and community integration. Qualifications Experience working with individuals with developmental disabilities preferred. Knowledge of DDA, TennCare, ECF CHOICES, or community-based services preferred. Understanding of survey processes, compliance expectations, and documentation standards. Strong organizational and time-management skills. Ability to multitask and work independently in a fast-paced environment. Excellent verbal and written communication skills. Professional and compassionate approach when working with individuals, families, and staff. Experience with scheduling, documentation, or program coordination preferred. Proficient in Microsoft Office and basic computer systems. Physical Requirements Ability to travel locally as needed for meetings, appointments, or individual support.
