Job Description
Job Summary The Resident Service Coordinator is responsible for ensuring residents receive appropriate care and services while maintaining their quality of life. This role coordinates individualized service plans, monitors resident needs, and serves as a key point of contact between residents, families, staff, and external providers. The coordinator supervises and works closely with the care team to address changes in condition, support compliance with regulations from the Texas Health and Human Services, and promote a safe, supportive living environment.
Key Responsibilities:
Conduct resident assessments and assist with service plan development Coordinate care and communicate with families and healthcare providers Maintain accurate documentation and ensure regulatory compliance Support resident engagement and overall well-being Collaborate with interdisciplinary team membersQualifications:
Experience in senior living, healthcare, or social services preferred Strong organizational and communication skills Knowledge of assisted living regulations in Texas is a plusPay:
$19.00- $25.
