Job Description

The Youth Program Manager leads the Transition Age Youth (TAY) Innovation Project, overseeing both the day-to-day operations and long-term success of the program. This role combines hands-on, housing-focused case management with full program management, including coordination, compliance, partnerships, and continuous improvement. This position works directly with youth experiencing or at risk of homelessness while also managing program implementation, strengthening cross-system collaboration, and ensuring services are responsive to youth voice and real-world needs. The ideal candidate is someone who can build trust quickly with young people, stay highly organized, and confidently manage a program from both a service and systems level. Key Responsibilities Program Leadership and Implementation Lead the overall implementation of the TAY Innovation Project, ensuring services are delivered consistently, effectively, and in alignment with grant requirements and program goals. Own day-to-day program operations, including workflows, service delivery processes, and coordination across internal and external partners. Ensure all grant deliverables, reporting requirements, and performance targets are met on time and with accuracy. Monitor program progress and outcomes, using data to identify gaps, improve services, and strengthen impact. Maintain compliance with funding requirements, documentation standards, and agency policies (including HMIS or comparable systems). Coordinate case conferencing and team meetings to support coordinated, youth-centered service planning. Work closely with leadership to ensure the program is operating effectively and aligned with broader agency priorities. Case Management and Direct Services Provide housing-focused, strengths-based case management to transition-age youth experiencing or at risk of homelessness. Conduct assessments, develop individualized service plans, and support youth in setting and achieving housing, education, employment, and life stability goals. Maintain an active caseload and provide consistent follow-up, check-ins, and ongoing support. Support youth in navigating housing applications, lease agreements, and landlord relationships. Connect youth to mainstream resources including employment, education, healthcare, behavioral health, and other supportive services. Maintain accurate, timely, and complete documentation in required data systems. Community Partnerships and Systems Coordination Build and maintain strong working relationships with schools, foster care systems, juvenile justice, reentry programs, and other youth-serving partners. Serve as a connector between youth, LHH services, and external systems to reduce barriers and improve access to housing and support. Collaborate with the Fredericksburg Regional Continuum of Care (FrCoC) and other regional partners to align with broader housing and youth strategies. Strengthen referral pathways and improve coordination between agencies serving transition-age youth. Youth Engagement and Leadership Develop and support a youth-led lived experience cohort that informs program design and improvement. Facilitate regular youth engagement and coalition meetings. Ensure meaningful youth participation by addressing barriers such as transportation, scheduling, and compensation. Incorporate youth feedback into program decisions, service design, and evaluation efforts. Advocacy and Systems Improvement Advocate for youth-centered, equitable approaches that reduce barriers to housing and services. Elevate youth voice in program design and broader system conversations. Identify gaps in services and contribute to solutions that improve coordination and outcomes for youth in the region. Minimum Qualifications 3-5 years of experience in case management, housing services, youth services, or related human services work. 2 years of experience managing or significantly supporting the implementation of a program, including responsibility for deliverables, grant and financial reporting, or outcomes. Strong experience providing housing-focused or strengths-based case management. Ability to manage both direct service responsibilities and program-level coordination simultaneously. Strong understanding of trauma-informed care, positive youth development, and motivational interviewing. Demonstrated ability to stay organized, manage deadlines, and independently move work forward. Strong communication skills with the ability to build trust with youth and collaborate effectively with partners. Experience working with diverse and historically marginalized populations. Proficiency in Microsoft Office and experience with HMIS or similar data systems. Valid driver's license and reliable transportation required. Must pass required background checks. Preferred Qualifications Experience with grant management and oversight, including compliance, performance tracking, data reporting, and funder communication. Demonstrated success in grant writing, proposals, or securing program funding. Strong background in program management, including launching or scaling initiatives, managing deliverables, and driving outcomes. Bilingual or multilingual skills, particularly in languages relevant to the local community. Experience facilitating youth engagement, youth leadership development, or lived-experience advisory groups. Work Environment Full-time position (40 hours/week). Flexible schedule required, including occasional evenings and weekends based on youth and program needs. Local travel required for meetings, outreach, and partner coordination.
