Job Description
Care Manager Mentor Job Responsibilities:
Provides on-the-job training, orientation and feedback to new Care Managers related to their assigned caseloads and job tasks. Serves as an on-going resource to Care Managers. Regularly communicates with the direct supervisor information about the new employees' progress, needs for improvement and/or educational training. Participates in efforts to enhance work experiences. Coaches staff to achieve the highest level of evidence-based practice. Leads and supports to enhance employees' work experiences and retain talent. Responsible for all job specific responsibilities listed in the Care Manager job description for Family Care or Family Care Partnership, as appropriate.Care Manager Mentor Job Requirements:
Education:
Bachelor's Degree in Social Work, Psychology or Human Services and 18 months experience with older adults or persons with physical and/or intellectual disabilities in a social service capacity OR a Masters' Degree in Social Work, Psychology or Human Services.Certification:
Long-term care functional screener certification is requiredTravel:
Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.Care Manager Mentor Job Schedule:
