Job Description

Silver Club Memory Programs is on one of the Community Health Services Programs located at the Turner Senior Resource Center (2401 Plymouth Rd). Silver Club provides activities and socialization for individuals with mild to moderate dementia. This position is a client facing position. We are looking for someone to join our team with client and event coordination skills and desire to work with older adults with dementia and their families. This position is hybrid. Hours are Monday through Friday 9am to 5pm. The Silver Club Memory Programs (SC) Project Coordinator will support SC?s efforts to provide coordination of community caregiver events, social support groups, and respite assistance for informal/formal caregivers taking care of a person living with dementia. This position will administer special projects and activities for caregivers and persons with dementia, prepare reports, prepare project plans, maintain records, develop program materials, respond to requests for caregiver support, coordinate with trainers and vendors, and ensure timely and accurate responses to customers and community partners. The Project Coordinator will support colleagues in facilitating care plans. Conduct community outreach. Position will serve as a resource liaison. This individual works collaboratively across teams, contributing to a supportive and coordinated environment, and remains adaptable in a dynamic, fast-paced setting. Support planning and execution of Silver Club Memory Programs caregiver events (internal and external), workshops, and support groups. Coordination and Implementation of the SC Coffeehouse education support program for persons recently diagnosed with dementia. Develop monthly caregiver event calendars, meeting logistics, and preparation of materials. Coordinate event logistics, including scheduling, venue coordination, materials preparation, catering, and registration tracking. Assist in tracking event budgets and vendor coordination. Support follow-up communications and post-event documentation. Assist in developing, implementing, and ensuring accuracy of Care Support Plans (CSP) for each person supported by moderate day care programming and their caregivers. Serve as a resource liaison for caregivers. Creates and monitors data tracking forms for program data collection. Submit reports to internal and external agencies. Maintain shared files, documentation systems, and tracking tools. Support procurement processes and monitoring the budget of projects working in conjunction with the Director. Conduct community outreach. Provide onsite activity support for Early stage and Moderate Programming as needed. Ability to travel to events throughout Washtenaw County.

SUPERVISION EXERCISED
Functional supervision may be exercised over students, interns, and volunteers Bachelors degree in health education, public health, social work, healthcare administration or similar. 2-3 years of experience in working with older adults, and implementation of community events. Demonstrated ability to meet deadlines and achieve project objectives? Demonstrated ability to work collaboratively as part of a team Proficiency with standard office and collaboration tools (e.g., Microsoft Office or Google Workspace), including document management and shared systems This position requires travel to events throughout Washtenaw County 3+ years of experience in working with older adults, caregivers, coordination and implementation of community events. Experience utilizing the UM FIN system, M-Reports, and Emburse. This position is hybrid. Hours are Monday through Friday 9am to 5pm.
