The Lead Care Manager oversees the provision of ECM services and implementation of the care plan, conducts outreach to assigned ECM members, engages them in enrolling into ECM, and delivers ECM services. Enhanced Care Management services are provided where the member lives, seeks care, in the office, and in the community.
Qualifications:
Highschool diploma or GED required. Associate of Science/Medical Assistant Certificate or Degree preferred. Knowledge of community resources preferred. Bilingual required. A valid California driver's license is required of staff who have regular driving duties, and the driving record must meet the requirements of their position and be approved by the agency's insurance company.
Essential Job Functions:
Operate an agency or personal vehicle for work-related activities which may include, client visits, meetings, trainings, or other agency-related appointments Ensure compliance with driving requirements, including maintaining a valid driver's license, current vehicle registration, and proof of insurance as required by agency policy. Ability to communicate ideas in oral and written form. Ability to instruct in group and individual settings. Physical, mental, and emotional health adequate to perform duties. Must possess knowledge of mental health programs and community resources. Adhere to all applicable legal and ethical standards Protect and honor individual integrity through confidentiality. Ability to work in a harmonious manner with subordinates, supervisors, and others. Ability to work effectively with cultural/ethnic diversity.
Responsibilities:
Provide training and guidance to frontline staff. Implement department/program objectives. Promote a healthy working environment that values staff and equips them to succeed while making sustainable decisions in a way that strengthens both our workforce and our impact. Responsible for executing organizational strategies and initiatives. Engage and enroll eligible members in ECM. Oversees provision of ECM services and implementation of the care plan. Utilize motivational interviewing, trauma-informed care and harm-reduction strategies. Responsible for coordination of services to ensure a seamless experience and unduplicated services. Advocate on behalf of members with health care professionals. Connect members to other social services and support including transportation. Provide Community Outreach and attend Community Events. Provide health promotion and self-management training. Accompany members to office visits, as needed. Monitor treatment adherence. Coordinate with hospital staff on discharge plans. Responsible for preventing and controlling infection.\ Responsible for maintaining a culture of quality and safety. Effectively use de-escalation skills and techniques for crisis prevention. Properly administer the C-SSRS to help prevent suicide.
Other:
Attend all required meetings and trainings. Report any suspected child or dependent adult/elder abuse or neglect immediately to direct supervisor or utilize the chain of command if supervisor is unavailable. Report any client imminent danger to self or to others or gravely disabled immediately to direct supervisor or utilize the chain of command if supervisor is unavailable. Other duties as assigned.
