Job Description
Peace River Center seeks a FACT Team Leader to guide a multidisciplinary Forensic Assertive Community Treatment team serving adults with serious mental illness and justice involvement. The Team Leader oversees daily operations, coordinates individualized treatment and recovery plans, and ensures high-quality, trauma-informed, and culturally competent services in home and community settings. Responsibilities include staff supervision, scheduling, crisis response coordination, outcome tracking, and collaboration with courts, hospitals, and community partners. This role supports a mission-driven, client-centered culture focused on empowerment, safety, and long-term community stability.
