Job Description

Elder Services of Berkshire County, Inc. Residential Coordinator & Case Manager Pittsfield, MA Apply Sign on bonus: \$1,500.00 Key Responsibilities Residential Coordination
  • Connect residents of all ages with community resources and supportive services, including assistance with applications for food benefits, transportation services, health insurance, and in-home support programs.
  • Collaborate closely with local housing authority staff to foster a safe, supportive, and inclusive living environment.
  • Plan and coordinate social, recreational, and educational activities to enhance residents quality of life and encourage community engagement.
  • Develop and distribute a monthly newsletter to inform residents about on-site programs, services, and community events.
  • Serve as a resource and point of contact for residents seeking information or assistance navigating community services. Case Management (Elder Services State Home Care Program)
  • Conduct comprehensive client assessments to identify needs and service gaps.
  • Develop and implement individualized service plans that support client independence and safety.
  • Arrange, coordinate, and monitor home care and community-based services.
  • Advocate for clients with service providers, agencies, and healthcare partners.
  • Maintain accurate case documentation and records in accordance with program and regulatory requirements.
  • Monitor client progress and adjust services as needed to ensure quality care.
