Operations Manager, Community Services Salary Range:

About the Role We're looking for a dedicated Operations Manager, Community Services to join our team and help us continue delivering the level of care our patients and families deserve. In this role, the Community Services Manager provides direct oversight of staff to ensure alignment with operational goals and patient needs. The Manager conducts regular 1:1 meetings and daily team huddles, leads case reviews, and coordinates case conferences to support high-risk, high-need Medicaid members and promote high-quality, patient-centered care. This role supports hiring and onboarding, staff training, and ongoing coaching while developing strategies that strengthen team performance and service delivery. In addition, the Manager develops and presents reports that measure efficiency, workflow, and quality metrics to inform decision-making and program improvement. Working collaboratively with department leaders, the Manager serves as a liaison with internal and external stakeholders to advance program goals, support community service planning, and improve patient outcomes while ensuring NYS Health Home and 1115 Waiver services remain compliant with NYS Department of Health and Saratoga Hospital requirements. This role is responsible for fostering and maintaining a team culture grounded in kindness, respect, and collaboration. What You'll Do Oversee the performance of the Community Care Management team, including supervision, performance appraisals, goal setting, training, and staff development. Initiate, implement, and monitor policies and procedures aligned with NYS DOH Health Home and 1115 Waiver requirements and care coordination best practices. Monitor program performance and ensure quality, efficiency, and performance metrics are accurately collected, evaluated, and reported in a timely manner. Lead quality and performance improvement efforts, including incident tracking, monitoring, and resolution. Collaborate with internal departments and external partners to promote continuity of care, cross-sector collaboration, and improved patient outcomes. Utilize knowledge of federal, state, and local resources to guide appropriate referrals that address patient unmet needs; assist with home visits as necessary. Establish and maintain relationships and contracts with network providers, community agencies, and program stakeholders. Participate in regional and statewide workgroups as assigned. Ensure full compliance with NYSDOH requirements and all regulatory standards. Maintain required documentation, statistics, and reports to meet program and regulatory reporting requirements. Complete projects within established timeframes and meet regularly with the Program Director to provide updates and address questions or concerns. Maintain confidentiality of all patient and program information.What You Bring Required Qualification:
Bachelor's degree and a minimum of one year of supervisory experience within the NYS Health Home program. Demonstrated success leading teams, managing program operations, and achieving performance and quality goals.Preferred Qualifications:
Extensive knowledge of the NYS Health Home program Licensed level healthcare professional with prior experience in a behavioral health setting or Master's level professional with two years of supervisory experience in a behavioral health setting.Licensed level healthcare professional includes:
Physicians, Psychiatrists, Physician's Assistants, Nurse Practitioners, Psychiatric Nurse Practitioners, Registered Professional Nurses, Licensed Practical Nurses, Licensed Psychologists, Licensed Clinical Social Workers, Licensed Master Social Workers, Licensed Mental Health Counselors, Licensed Marriage and Family Therapists, Licensed Psychoanalysts, Licensed Creative Arts Therapists, and Licensed Occupational Therapists. Previous experience working with high-risk, high-need populations. Must be able to use Microsoft Outlook, Excel and other proprietary software. Must be able to pass background checks. Experience using Netsmart Care Manager, MAPP, PSYCKES and other NYS applications Knowledge of community resources Excellent customer service skills Able to work a flexible schedule.
