Identify and coordinate the goals/outcomes for each individual being served in cooperation with the individual and his/her identified team. Review service documentation on a regular basis, to help ensure quality compliance with the program and agency standards. Help ensure the highest quality of care to individuals that is helping them meet their needs. Help ensure that our services and operations are in compliance with Access Services' established policies and procedures. Maintain effective working relationships with representatives of state and local agencies and other professionals. Report on any operational, staff or individual concerns according to Access Services' established policies and procedures. Maintain appropriate, timely documentation as required of the position. Provide or prepare data such as statistical or financial information about the caseload. Keep abreast of developments in the field relating to the provision of services and intervention techniques. Provide direct care and direct support hours to meet program and individuals' needs, including but not limited to transferring, rotating, and changing individuals.
