Full-time, Exempt Position Summary:
The Program Manager will report to the Community Supports Program Director and support the implementation and scale of Partners' Community Care Hub (CCH) Private Duty Community Supports program, inclusive of Respite and Personal Care and Home Maker services. The Program Manager will provide leadership and general oversight of the day-to-day operations of the program's goals and objectives, including assessing program capacity and working with the CCH Liaison for recruitment and onboarding of new CCH subcontractors.
Minimum Qualifications:
- B.A. in human development, social work, psychology, counseling, rehabilitation, gerontology, sociology or a related field. Masters-level preferred.
- Minimum of 5 years of experience working in healthcare or multidisciplinary community-based settings providing case management or care coordination services.
- Experience working with diverse individuals and families experiencing health related social needs, adults/individuals with disabilities populations of focus preferred.
- Minimum of 3 years of supervisory experience
- Bilingual preferred

Responsibilities:
1. Manage and provide team oversight over daily operations of the Private Duty Community supports service line. 2. Maintain strong Community Care Hub Provider partnerships. 3. Manage provider capacity, escalations, complaints, compliance and oversight of Managed Care Plan (MCP) metrics and ensure appropriate reporting. 4. Ensure the documentation and reporting of Hub member roster, capacity, escalations, complaints, and performance, is conducted in a timely manner and identify areas for improvement. 5. Supervise Community Support Supervisor and team of CS Subject Matter Experts. 6. Work across departments to ensure teams are aware of private duty services and collaborate to facilitate cross-referrals 7. Collaborate with leadership to streamline workflows and assess staff capacity and performance. 8. Collaborate with Partners' Quality Team to support implementation and dissemination of quality assurance standards and initiatives. 9. Attend regularly scheduled meetings, training, and other meetings as identified that support Partners' services and programs. 10. Support with regular team check-ins and report submissions, ensuring performance and goals are on track. Partners in Care Foundation is an equal opportunity employer. Pay:
$70,304.00 - $83,000.00 per year
