Help participants navigate challenges, troubleshoot barriers, and build the skills and confidence to independently access resources over time. Coordinate with NWTC staff and community partners to support participants while avoiding duplication of existing services. Maintain timely documentation of participant goals, interactions, referrals, progress, and outcomes in the program's data system. Identify recurring service gaps or systemic barriers experienced by participants and share insights with BCUW and partners to strengthen the pilot and improve service coordination.
