Elmira Recuperative Care provides short-term medical respite and recuperative care services for individuals experiencing homelessness after hospital discharge. We partner with hospitals, health plans, and community organizations to help members recover safely while transitioning to permanent housing and improved health outcomes. As we continue to grow, we are seeking an experienced CalAIM Program Director to oversee our Community Supports and Recuperative Care operations.

The CalAIM Program Director will lead the strategic development, implementation, and oversight of Elmira's CalAIM programs. This individual will ensure compliance with DHCS requirements, health plan contracts, and quality standards while building strong relationships with hospitals, managed care plans, and community partners. This leadership role is responsible for program performance, staff supervision, compliance, operational excellence, managing authorizations and renewals and continuous quality improvement. Responsibilities Oversee daily operations of 3 recuperative care facilities Ensure compliance with DHCS, Medi-Cal Managed Care, and health plan requirements. Develop and maintain policies, procedures, workflows, and documentation standards. Supervise and mentor case managers, housing navigators, site managers, and administrative staff. Monitor program outcomes, KPIs, quality measures, and reporting. Build relationships with hospitals, managed care organizations, and referral partners. Oversee authorizations, extensions, audits, and documentation quality. Prepare for health plan and regulatory audits. Implement quality improvement initiatives and performance monitoring. Assist with program expansion, new contracts and strategic planning. Collaborate with executive leadership on operational and financial goals. Maintain compliance with HIPAA and all applicable state and federal regulations. Preferred Bachelor's degree in Healthcare Administration, Public Health, Social Work, Nursing, or related field. Minimum 3 years of leadership experience in CalAIM, Community Supports, ECM, Recuperative Care, Case Management, or Medi-Cal programs.
Strong knowledge of:
DHCS CalAIM Community Supports Housing Navigation & Housing Tenancy Services Recuperative Care / Medical Respite Excellent organizational and leadership skills. Strong written and verbal communication. Experience working with hospitals, health plans, or homeless services.
