Director of Social Services Summary of Duties and Responsibilities:
Foothills Compassionate Care bereavement program under Hospice of the Foothills (HOF): The Bereavement Coordinator is responsible for providing and coordinating bereavement services for hospice patients, families, caregivers, and the community. This position oversees the day-to-day operations and development of the HOF Bereavement program and services involving support groups, volunteer training, dying vigils, outreach, and events.
Job Details and Essential Functions:
  • Provides and coordinates bereavement assessments on patients, family, and caregivers and logs into the EMR system.
  • Provides brief counseling and/or referrals to the bereaved as necessary.
  • Plans, schedules, coordinates, screens participants, and facilitates bereavement support groups. Oversees support group facilitators.
  • Coordinates with other departments for community outreach, Celebration of Life planning and community engagement and education.
  • Participates in development/review of policies and procedures as they pertain to bereavement services.
  • Ensures compliance with State and Federal regulations and ensures ethical bereavement practices.
  • Maintains accurate tracking and correspondence systems and processes of bereavement groups, contacts, and mailings.
  • Develops and maintains systems for tracking volunteer activities and hours.
  • Educates staff on ongoing bereavement resources, education, and updates for increased knowledge.
  • Consults and collaborates with HOF staff and community referral agencies concerning bereavement services.
  • Participates in training, coordination, and evaluation of bereavement volunteers.
  • Participates in HOF orientation and in-service programs.
  • May assign and supervise people and activities associated with bereavement programs and services.
  • Performs administrative functions such as audits, observation of bereavement groups, clinical records, care plans and related.
  • Sends participant surveys at regularly established intervals. Maintains, manages and reports data and statistics for bereavement related activities and survey scores.
  • Works with Director of Social Services in creating and operating within the budget allocated for the bereavement program.
  • Participates in performance improvement projects aimed at the quality and safety of services provided by the agency and reports outcomes in the Quality Assurance Performance Improvement meetings.
  • Performs other assignments as appropriate.
Knowledge and Skills:
Required
  • Must be able to relate well to death and dying life situations and challenges to support the hospice participants and community.
  • Must have excellent organizational skills and ability for multi-tasking. Must be able to complete required paperwork with accuracy and meet prescribed deadlines.
  • Ability to manage, assign and evaluate bereavement volunteers for both patient and organizational needs.
  • Ability to develop and deliver educational in-services and trainings.
  • Knowledge and skill with computer software programs to include Microsoft Suite and other software for tracking and organizing and planning. Desired
  • Public speaking, developing relationships with identified partners, groups, businesses and community programs.
Education and Experience:
Required
  • BA or BS in social services or equivalent theological degree.
  • Minimum of 1 year experience and/or education in grief or loss counseling or three (3) or more years of experience in training and supporting volunteers in a health care, human services or similar agency. Desired
  • Supervisory experience and/or the ability to train and evaluate employees, volunteers, programs, and interns preferred.
  • MA or MS in a counseling or mental health field, or related field.
